Enhancing the Community | Infoxchange
Australian not-for-profit Infoxchange has developed the Find My Way Portal for Western Australia’s homelessness system in partnership with the Department of Communities WA, Anthologie, Anglicare WA and Third Story.
The Digital Front Door and OneStory Database reduce fragmentation, trauma, and repeated assessments through a human-centred design approach with extensive lived experience involvement throughout development. Though operational for under 2 months, the system is already showing promising early adoption with service providers reporting decreased need for follow-ups with referrals, improved access to services with less frustration, and enhanced collaboration through real-time data sharing.
